Project objectives
- To develop and pilot a protocol for the management of ePROMS across a range of entry points, inclusive of monitoring, outcome and evaluation: day infusion, Genesis Care, surgical preadmisson and non-admitted cancer patients.
- To improve completion rate and frequency of SCSTs.
- To improve timeliness and coordination of care following completion of the SCSTs, facilitateted by Wellness Coordinators (WC)
