Long Term Follow-up Program recommendation implementation project
The PICS Long Term Follow-up Program (LTFP) service model has been redesigned and enhanced through the implementation of 37 consumer-informed recommendations, ascertained from a 2020–21 service review.
The recommendations centred on:
- improving consistency and quality of care throughout the survivorship journey
- increasing program capacity to meet increasing demand
- enhancing care provider capability to meet patient needs
- greater collaboration with program partners
- empowering patients and families through the development of consumer informed resources.
Outputs
19 of the 37 recommendations had been fully implemented by July 2023. Another 14 are underway, many to be addressed through:
The remaining 4 recommendations will be addressed through the PICS program of work, under the following themes:
- addressing unmet need in fertility information and support
- development of the general practitioner (GP) and paediatrician shared-care model.
An upcoming evaluation of the recommendation implementation will include assessment of current patient and family experiences, compared to 2020–21, and use of new key performance indicator dashboards to ensure continuous improvement.
