Survivorship care plans at Northeast Health Wangaratta

A project to better support patients transitioning from end of active treatment into self-management/survivorship care. Providing patients with survivorship care plans to assist and support in addressing their patients’ health and wellbeing needs beyond active treatment. Evaluation has been completed and recommendations include scaling up to all tumour streams.
